Most common causes
Outdated plugin or theme
The vast majority of infections get in through an add-on with an unpatched hole. Bots scan the internet and exploit it en masse, often on the day it is disclosed.
Pirated ("nulled") add-on
Plugins and themes from illegal sources almost always have a backdoor built in. It is the most common hidden cost of a "free" premium plugin.
Weak or stolen password
A guessed password to the panel, FTP, or database gives the attacker full access without any sophisticated exploit.
Leftover backdoor
After the first breach the attacker leaves a hidden door. That is why just "cleaning" the files without finding it makes the infection come back.
